Common permit fees include the Controlled Substance Permit ($225.00 new / $150.00 renewal, plus a $10.00 state PDMP fee), the Oral Conscious Sedation Permit ($200.00 new / $100.00 renewal), the Parenteral Sedation and General Anesthesia Permits ($1,350.00 new / $300.00 renewal each), and the Hygiene Infiltration Anesthesia Permit ($100.00 new / $75.00 renewal). A duplicate or replacement license costs $25.00, dental license reinstatement carries a $250.00 penalty, and dental hygiene reinstatement carries a $100.00 penalty.

Tip Licenses renew annually. Renewals open August 1; all continuing education for the year must be uploaded to CE Broker and all fees paid before renewing.
