Utah Dental License Renewal

Fees, deadlines, and what's required to renew a dental license in Utah.
Utah dental licenses renew on a two-year cycle, due May 31 of even-numbered years, through My License One.

These are drawn from DOPL's division-wide Licensing and Miscellaneous Fees schedule. DOPL does not publish a separate itemized initial-application (new licensure) fee for each dental credential on its dentistry pages; the applicable fee is calculated within the My License One portal at the time of application.
How to renew
- Log in to My License One using your UtahID (create one if you don't have one).
- In the "Existing License/Registration Holders" section, find your license and click the three dots under "Actions" to start renewal.
- A receipt is emailed within 2 business days; the renewed license is emailed 1–3 business days after approval. Fees are non-refundable.
Active-duty military members and spouses may qualify for a Military Fee Waiver by emailing the waiver form, license renewal form, and a copy of active military orders to b9@utah.gov.
