Fees, deadlines, and what's required to renew a dental license in Arizona.

Arizona dental license fees and renewal periods are set by statute and processed through the Board's online portal at azus-sbde.ongovcore.com/public. Under A.R.S. § 32-1207, dentists, dental hygienists, and dental business entities renew every three years, with the license expiring on the licensee's birthday rather than a fixed date.

A $100.00 late renewal fee applies to any license not renewed on time (ARS §§ 32-1236(D), 32-1287(D), 32-1297.06(D)). Dentists with an anesthesia and sedation permit also pay $300.00 to obtain or renew the permit, plus $25.00 for each additional office location (ARS § 32-1207(E)).

Refund policy Jurisprudence examination fees are nonrefundable. All other fees may be refunded only if there was an error in payment — such as an incorrect or duplicate payment — or the Board is unable to fulfill a request that required a fee. Refund requests must be submitted in writing.
