Oklahoma Dental License Renewal

Fees, deadlines, and what's required to renew a dental license in Oklahoma.
Unlike states with multi-year renewal cycles, Oklahoma dental licenses and permits are renewed annually — every license and permit expires December 31 each year, regardless of when it was issued. A law change effective November 1, 2012 eliminated the prior grace period: renewals postmarked after December 31 are considered expired, and it is a violation of the State Dental Act to practice on an expired license, though the Board will still accept a renewal with the late fee through March 31.

Additional fees: a Professional Entity (trade name) registration is $20.00 per owner, per entity, and applies only to in-state entities; a Facility or Provider anesthesia permit application is $100.00 and requires an on-site inspection before the permit is issued.
