| Dentist — renewal application fee | $296.40 |
| Dentist — relicensure (lapsed license) | $342.40 |

| Registered Dental Hygienist — renewal application fee | $78.00 |
Fees for other credential types (Dental Therapist, Registered Dental Assistant, and the limited/specialty dentist licenses) are published in each credential's licensing guide PDF on the LARA dentistry page.
Relicensure after a lapsed license
Licensees whose license lapsed more than 3 years but less than 5 years, and who have not held a valid license in another state or Canada in the 3 years before applying, must retake and pass the ADEX clinical and written exam. Licensees lapsed 5 years or more under the same conditions must retake the full examination sequence required for initial licensure by examination.
Renewal also requires accepting a renewal attestation certifying completion of continuing education (see below), a one-time human trafficking training certification, and — for those who answer "yes" to the disciplinary or good-moral-character questions — supporting documentation.
