Michigan Dental License Renewal

Updated August 2026

Fees, deadlines, and what's required to renew a dental license in Michigan.

Michigan dentist and dental hygienist licenses renew on a 3-year cycle. Licensees can begin renewing online through MiPLUS up to 90 days before their license expires, and are responsible for renewing on time even if they do not receive a renewal notice — licensees must notify LARA in writing of any address change.

License type Renewal / relicensure fee
Dentist — renewal application fee $296.40
Dentist — relicensure (lapsed license) $342.40
Controlled Substance License — renewal $248.10
Registered Dental Hygienist — renewal application fee $78.00

Fees for other credential types (Dental Therapist, Registered Dental Assistant, and the limited/specialty dentist licenses) are published in each credential's licensing guide PDF on the LARA dentistry page.

Relicensure after a lapsed license

Licensees whose license lapsed more than 3 years but less than 5 years, and who have not held a valid license in another state or Canada in the 3 years before applying, must retake and pass the ADEX clinical and written exam. Licensees lapsed 5 years or more under the same conditions must retake the full examination sequence required for initial licensure by examination.

Dental amalgam separators Dentists subject to Administrative Rule 338.11811 must verify at renewal that they have installed and are operating a compliant dental amalgam separator, including the separator's make and installation year. The rule does not apply to oral and maxillofacial surgeons, oral and maxillofacial radiologists, oral pathologists, orthodontists, periodontists, dentists practicing in a dental school, hospital, or local health department, or dentists who use a holding tank and do not discharge amalgam waste.

Renewal also requires accepting a renewal attestation certifying completion of continuing education (see below), a one-time human trafficking training certification, and — for those who answer "yes" to the disciplinary or good-moral-character questions — supporting documentation.
